Case Management for the Elderly

Elderly Waiver is a special program which provides a variety of services to keep persons 65 years of age or older in their homes longer and safer.

How do I know if I qualify?

  • Am I 65 years of age or older?
  • Is my monthly income less than $1,911?
  • Do I have less than $2,000 in assets (excluding my home, car and burial contracts)?
  • Do I have difficulty taking care of myself?

When a person meets all of the above qualifications, a case manager will work with the individual to coordinate needed services.

Examples of service include:

  • Homemaker
  • Lifeline
  • Home delivered meals
  • Transportation
  • Adult Day Care
  • Mental health outreach
  • Assistive devices
  • CHORE services (lawn moving, raking, snow removal)
  • Liquid supplements

Medicaid coverage may also be included to help with medical and prescription costs.

For more information, call 319-653-7758 or 800-655-7758.
