Case Management for the Elderly
Elderly Waiver is a special program which provides a variety of services to keep persons 65 years of age or older in their homes longer and safer.
How do I know if I qualify?
- Am I 65 years of age or older?
- Is my monthly income less than $1,911?
- Do I have less than $2,000 in assets (excluding my home, car and burial contracts)?
- Do I have difficulty taking care of myself?
When a person meets all of the above qualifications, a case manager will work with the individual to coordinate needed services.
Examples of service include:
- Homemaker
- Lifeline
- Home delivered meals
- Transportation
- Adult Day Care
- Mental health outreach
- Assistive devices
- CHORE services (lawn moving, raking, snow removal)
- Liquid supplements
Medicaid coverage may also be included to help with medical and prescription costs.
