Blood Pressure Clinics

 Blood Pressure Clinics are offered monthly at various sites throughout Washington County by Washington County Public Health & Home Care.  Any adult is welcome and there is no charge.  See Schedules for places, days of the month and times.

 

Case Management

 Case Management is a program over the entire state that provides coordination of services to all Iowans over the age of 60 at no charge.  A Case Manager will assist the client in finding services they need.  There are no income or asset guidelines.

Elderly Waiver for the Frail Elderly

  Elderly Waiver for the Frail Elderly is a special program under Case Management which provides a variety of services to keep persons over the age of 65 in their homes longer and safer.  Clients have to meet level-of-care and financial guidelines.  Income must be under $1,809/month for one person.  Services include:  homemaker, Lifeline, home delivered meals, frozen meals, groceries, transportation, adult day care, mental health outreach, care giver support, respite, assistive devices, chore service, and liquid supplements.  A total of $1,052 may be spent monthly on services.  Medicaid is also included to help with medical and medicine costs.
