This is located at the United Church of Crawfordsville in Crawfordsville! We will be accepting Medicare (no Medicare supplement plans) and private pay, which will be $35. If you do private pay, we can give you a receipt to send in to your insurance for reimbursement. We can't wait to see everyone!
